Permit to Administer Neuromodulators application
Any PEIDC registrant who plans to administer pharmaceuticals such as neuromodulators (e.g. Botulinum Toxin Type A) and adjunctive non-surgical and/or surgical therapies must have a valid Neuromodulator Dentist Authorization from the PEIDC. The permit is subject to annual renewal.
Treatment levels
There are three treatment levels related to neuromodulators:
- Level 1—Applied anatomy review and introduction to neuromodulators (does not require a permit)
- Level 2—Basic neuromodulators: upper face and bruxism treatment—permit application fee: $200
- Level 3—Advanced neuromodulators: mid-face and lower face/neck regions—permit application fee: $400

Application
Please follow these steps to apply for a Permit to Administer Neuromodulators:
- Download and fill out the application form
- Using the contact form, submit:
- Filled-out application form, and
- Proof of completed training (when applying for the first time)
- Pay the application fee (payment instructions are included in the application form)
Permit to Administer Neuromodulators is subject to annual renewal during the renewal period.
