Facility Sedation Permit application
All dental facilities practicing sedation must apply for a Facility Sedation Permit. The permit must be renewed annually, by March 31.
There are two permit levels:
- Level 1—nitrous oxide and oxygen sedation only—application fee: $150
- Level 2—parenteral moderate, or deep sedation/general anesthesia—application fee: $300
Primary contact
Each dental facility must select the primary contact—a general dentist or dental specialist—who will apply for the permit on the facility's behalf. The primary contact and all practitioners listed in the form must be registered with the College and authorized to administer the appropriate level of sedation.
Application
Please follow these steps to apply for a Facility Sedation Permit:
- Download and fill out the application form
- Submit the completed document using the contact form
- Make sure the appropriate permit fee is paid
